P308 - Integrated DSMES Multidisciplinary Clinic to Improve Outcomes in a Family Medicine Residency Clinic

Pharmacists are important members of a multidisciplinary diabetes care team and can provide essential clinical and educational services. This poster reviews a study to assess the effect of a pharmacist-led multidisciplinary care clinic on hemoglobin A1c (A1c) reduction in comparison to usual care at the same site. A pharmacist was integrated into a family medicine residency multidisciplinary care team to provide clinical and diabetes self-management and education services. In addition to A1c, this study also evaluated the difference in health maintenance completion, number of in-person and telephone encounters, diabetes-related hospital encounters, use of evidence-based therapies, and weight loss.
